Patient Referrals
Patients in need of services must be referred
by their physician for all therapies. Many managed care insurance carriers
require a written referral/authorization form from the patient’s primary
care physician and/or insurance carrier prior to the start of therapy.
To keep physicians informed of their patient’s progress, SJMO provides monthly progress reports as well as weekly updates to your physician.
