Patient-Centered Medical Home: A Partnership between Patient and Physician

Many of our primary care offices are Patient-Centered Medical Home partnerships between the physician and his/her team and you as an informed patient. This trusting relationship serves as a "home base" for all your health care needs through all stages of your life. You have one place to call; you have greater access to services, you receive care personalized to your needs, and the care focuses on wellness and preventing disease to keep you healthy and active.

What You Can Expect from US. WE will:

  • Listen to you-help you set your health care goals as we focus together on disease prevention.
  • Give you clear information, explanations and directions about medicines we prescribe and treatment we provide.
  • Listen to your questions and answer them clearly.
  • Guard your privacy.
  • When necessary, send you to specialists we know and trust.
  • Provide 24-hour access to medical care and same day appointments.
  • Tell you how to meet your health care needs when the office is closed.
  • Strive to offer scheduling flexibility.
  • Respect you as an individual.
  • Give you instructions at the end of every visit about treatment goals, expectations, medicines and future plans.
  • Base all care on quality and safety.

  • Communicate openly about your symptoms.

  • Take all of your medications and follow your physician's advice.
  • Call the office first with all health care problems or concerns, unless it is an emergency.
  • Inform us when you see other doctors and about changes in medication.

Appointments
We offer same day appointments.

Please arrive five to 10 minutes before your scheduled appointment time and sign in at our reception desk. If you are unable to keep your scheduled appointment, please notify us as soon as possible.

Prescriptions and Refills
Please ask us to refill your prescription at the time of your office visit.

If you are concerned that your medication will run out before your next visit, please call the office at least 24 hours in advance of when you need the medication refill. Please have the name of your prescription, the dose and your pharmacy's telephone number available.

After Hours and Urgent Care
If you have health concerns after regular practice hours, a health professional will be available via telephone at 248-858-6959 through St. Joseph Mercy Oakland.

After hours, we may direct you to an urgent care center for treatment. St. Joseph Mercy Oakland offers urgent care in three convenient locations:

About Timothy G. Dickson, MD

Dr. Dickson received his undergraduate education at Michigan State University and attended medical school at Wayne State University. He completed his residency at Wayne State University/Detroit Medical Center. He is board-certified by both the American Board of Internal Medicine and the American Board of Pediatrics. He is a member of the American Medical Association, American Academy of Pediatrics and the American College of Physicians.

 
     

About Theresa Larsen, MD

Dr. Larsen received her undergraduate education at Michigan State University, attended medical school at the University of Michigan and she completed her residency at William Beaumont Hospital. She is board-certified by both the American Board of Internal Medicine and the American Board of Pediatrics. She is a member of the American College of Physicians and is a fellow of the American Academy of Pediatrics.

 
     

About Amy J. McCarthy, MD

Dr. McCarthy received her undergraduate education from Delta College and Michigan State University. She attended medical school at Wayne State University and she completed her residency at William Beaumont Hospital. Dr. McCarthy is a clinical assistant professor in the Wayne State University Department of Medicine. She is board-certified by both the American Board of Internal Medicine and the American Board of Pediatrics.

About Walter Culver, MD

Dr. Culver received his undergraduate education from Michigan State University and his medical degree from Wayne State University. He completed his residency in both internal medicine and pediatrics from Wayne State University/Detroit Medical Center. Dr. Culver is a member of the American Medical Association and the American Academy of Pediatrics. He is board-certified in both Internal Medicine and Pediatrics.

About C. Kohler Champion, MD

Dr. Champion is board-certified by the American Board of Internal Medicine. He is a graduate of the University of Michigan Medical School and completed his internship and residency at the University of Michigan Hospital. He serves on the teaching faculty at St. Joseph Mercy Oakland. Dr. Champion is a member of the American Medical Association, Michigan State Medical Society, Oakland County Medical Society, Associates of Clinical Pharmacology, American Rheumatism Association, America Arthritis Association and Michigan Lupus Foundation.

 
     

About Michael S. Krivitsky, DO

Dr. Krivitsky completed his undergraduate education at Worcester State College in Worcester, Mass. and his medical education at the University of Health Sciences in Kansas City, MO. He completed his internship at the University of Texas in San Antonio, TX and residency at St. John Hospital in Detroit. He is board-certified by the American Board of Internal Medicine.

 
     

About Nadia J. Sadik, MD

Board-certified by the American Board of Internal Medicine, Dr. Sadik is a graduate of Al-Mustansiriyah University School of Medicine in Iraq. Dr. Sadik completed her internship at Medical City Teaching Hospital in Iraq and her residency at Marshall University School of Medicine in Huntington, WV.

About Stacey Gorman, MD

Dr. Gorman is board-certified in Pediatrics by the American Board of Pediatrics. Dr. Gorman's residency was performed at Children's Hospital of Michigan/Wayne State University. She is a member of the American Academy of Pediatrics.

Dr. Kowalski is board-certified by the American Board of Pediatrics. Her undergraduate education was complete at Wayne State University and her medical education was completed at Michigan State University College of Human Medicine. About Geetha Nair, MD

Dr. Nair is board-certified by the American Board of Pediatrics. Dr. Nair received her undergraduate education at Mount Carmel College and completed her medical education at Bangalore University Medical College, both in Bangalore, India. She performed her residency training at Kosair Children's Hospital, University of Louisville, KY. She is a member of the American Academy of Pediatrics, Association of Kerala Medical Graduates (AKMG) and Karnataka Medical Graduates Association.
